Welcome to the Confluent Health Virtual Clinic portal. You can schedule your virtual appointments for physical therapy and physician visits. You can schedule with physical therapists, orthopedic surgeons, neurosurgeons, pain medicine specialists, and primary care practitioners. We will get you scheduled quickly, usually within 24-48 hours.

How it Works

  • Virtual Physician Visits & Independent Medical Examinations (IME)

    A Confluent Health virtual visit is a live, two-way interaction with a patient and their provider. These visits are held through Microsoft’s Bookings platform and are 100% HIPAA compliant. This service is intended to serve as a substitute for an in-person encounter when circumstances limit the ability for a patient to travel to an appointment.

  • Virtual Physical Therapy

    Virtual physical therapy focuses primarily on treating conditions affecting the musculoskeletal system, including joints, muscles, bones, ligaments, and tendons. These are live, two-way interaction visits with a board-certified physical therapist and are geared towards guiding patients through strengthening, flexibility, balance, and functional mobility exercises to help them recover from injury or surgery.

    • Please wear comfortable clothes that allow for movement. We also ask that you have any prescriptions from your healthcare provider (if you have one) available along with any imaging or other reports.  If you have braces, splints, or crutches, please have them with you during your telehealth visit, as well.
    • During your first appointment, an expert Physical Therapist will complete a comprehensive evaluation, evaluating your current condition and allowing us to determine the most appropriate course of treatment.  Once a course of treatment has been established, your Physical Therapist will discuss with you a program to meet your goals and get you back to recovery. The appointment should take about 30 minutes.
  • Preparing for Your First Appointment

    These visits are held through Microsoft’s Bookings platform and are 100% HIPAA compliant.  You’ll need to download the TEAMs app on your smart phone, tablet, or computer.

  • Who will be in charge of my Physical Therapy?

    One of our Board-Certified Physical Therapists will complete a comprehensive evaluation virtually through live, two-way interaction, which will then be used to formulate a treatment plan.

    How do I join the virtual appointment?

    Once you have scheduled your appointment and downloaded Microsoft’s TEAMS application onto your smartphone, tablet or computer, you will receive an email invitation from your provider that will take you directly to your scheduled appointment.

    Before you can open the link, you’ll need to download the TEAMs app on your smart phone, tablet, or computer.

  • What should I wear for Physical Therapy?  Do I need to prepare anything?

  • What can I expect at my first Physical Therapy appointment?

    During your first appointment, an expert physical therapist will complete a comprehensive evaluation virtually, evaluating your current condition and allowing us to determine the most appropriate course of treatment.  Once a course of treatment has been established, we’ll also talk about incorporating home exercise programs to supplement your visits. The appointment should take about 30 minutes.

  • How long are the physical therapy visits?

    Your initial evaluation will be scheduled for one hour; the rest of your individual appointment times and durations vary dependent upon your personalized treatment plan.

  • What can I expect at my IME?

    An Independent Medical Evaluation (IME) is an examination like any evaluation from a physician but without recommendations for care.  An IME helps answer specific legal or administrative questions related to a variety of situations, e.g., a disability claim; workers’ compensation case; a personal injury lawsuit (tort claim); impaired professionals program; or sexual harassment in the workplace.

  • What should I do to prepare for my physician visit?
    To be prepared for your physician visit, be sure to have a medication list, a written medical and surgical history, a list of any allergies you have.  Also be prepared to answer specific questions about the reason for your visit.
